St. Patrick Home Care is an in-home care services that offers an alternative to Senior Living Facilities, Nursing Home, Boarding Care. Our service are offered in the comfort of your own home. We connect clients who have special needs with caregivers who have special skills.

St. Patrick Home Care is a family-owned and operated company situated in San Jose, California. We provide in-home care to the counties of Santa Clara, San Mateo,  Alameda and San Francisco.

With our mission of providing compassionate, personalized, hands-on, and client-centered care, we do everything we can to improve our clients’ lives and to uplift their family members.

Our staff members see to it that your loved ones are put first. They have passed all of the background, drug, and reference checks, personal interviews, and screenings needed to be able to execute the care plan we tailor to fit your loved ones’ wants and needs. We also require them to attend seminars and training sessions to hone their skills further.

Our Process

Here is what happens when you request St. Patrick Home Care to extend assistance to your loved ones:

  • One of our care coordinators is always available to talk to you about the needs of your love ones. Just give us a call and a care coordinator will speak with you on the phone, whichever you prefer.
  • We begin by conducting a non-medical assessment to determine the client’s level of functioning, needs, desires, and conditions of client and the family in order to establish what services will be provided through discussion with the client and involved family members.
  • Working in partnership with the family, we develop an individualized program to manage your daily needs and match your needs to the best possible caregiver, bring then to your home and ask you to participate in the selection process.
  • After the caregiver has been placed in your home, we continually communicate with the caregiver who is providing care to you and maintain our unique system of continued personalized contacts through telephone check-in or home visits.
